SCREENING FOR SAFE BFR TRAINING

Safety with BFR training is the top priority for researchers and clinicians alike. However, due to the novelty of the approach and the way in which training is performed through restriction of arterial flow, there are concerns regarding the likelihood of acquiring a blood clot when training with BFR. It can be helpful to not only have a framework for appropriate BFR candidacy (see our recent publication titled "Perceived Barriers to Blood Flow Restriction Training" in Frontiers in Rehabilitation Science - Open Access), but also a way to stratify risk. This chart was proposed in 2011 by Nakajima et al. as a way to help with the clinical decision-making process. This chart could be used in tandem with our screening algorithm to make the BFR screening process much more comprehensive.

PERCEIVED BARRIERS TO SUCCESSFUL BFR TRAINING

NEW paper publication! This one was in the works for a year but accurately reflects the major challenges that BFR trained providers go through following taking a continuing education course from @thebfrpros or @owensrecoveryscience. This paper also attempts to introduce a new screening algorithm to better determine appropriate candidacy for BFR training. Last, we propose relevant research questions that have yet to be systematically addressed in the literature with a hope to guide future research!

I want to say thanks to my co-authors for helping this one reach the finish line. Collaboration is what it's all about and I couldn't be more thrilled to contribute a little part to the BFR training literature.

When you end up in the same place with different types of training programs (Lixandrao 2018), how likely are the mechanisms underpinning the changes in growth truly different? I think we tend to overlook the forest for the trees. What mechanisms are likely present with both types of training despite the differences in acute loading parameters? I know we talk about metabolic stress, mechanical tension and muscle damage, but what factors are necessary for growth? It appears mechanical tension is the true link between ALL loading protocols, but the WAY in which you get to mechanical tension in the different loading protocols is what is different. All else - including the local responses (ie hormones, metabolic stress etc) are a function of the training stimulus...not the mechanism itself.
That is to say, metabolic stress at lighter loads is the way in which mechanical tension is augmented to produce growth whereas in heavy loads, mechanical tension is high already due to the heavy weight. Connect the dots. Occam's razor...simplicity is likely the most probable explanation.
Now this isn't to say that despite the similar mechanisms of mechanical tension driving the growth at various loading parameters we can't get unique benefits with BFR. For example, more frequent training protocols because of the unique stress given on the myofiber. Compared to heavy training where there is more systemic demand including muscle damage, cardiovascular stress etc., the potential for BFR to fit into a training program is insane.
What do you think about the explanation that mechanical tension can explain 95%+ of the mechanisms behind the muscle growth observed in BFR training?

Had a great day instructing 23 physical therapists of Twinboro Physical Therapy on the most up to date #BFRTraining information in North Brunswick, NJ. We were able to integrate our online course into the didactic material and I was able to expand upon each module afterwards to give a more nuanced take and answer live Q&As along with discussing our new recently accepted publication. It was a really good launch for this 9 hour hybrid training course - something no education company is doing (that I am aware of!).
